Recommended Breastfeeding Practices 1. Human milk is the preferred feeding for all infants, including premature and sick newborns, with rare exceptions.[75-77] The ultimate decision on feeding of the infant is the mothers. Pediatricians should provide parents with complete, current information on the benefits and methods of breastfeeding to ensure that the feeding decision is a fully informed one. When direct breastfeeding is not possible, expressed human milk, fortified when necessary for the premature infant, should be provided.[78,79] Before advising against breastfeeding or recommending premature weaning, the practitioner should weigh thoughtfully the benefits of breastfeeding against the risks of not receiving human milk. 2. Breastfeeding should begin as soon as possible after birth, usually within the first hour.[80-82] Except under special circumstances, the newborn infant should remain with the mother throughout the recovery period.[80,83,84] Procedures that may interfere with breastfeeding or traumatize the infant should be avoided or minimized. 3. Newborns should be nursed whenever they show signs of hunger, such as increased alertness or activity, mouthing, or rooting.[85] Crying is a late indicator of hunger.[86] Newborns should be nursed approximately 8 to 12 times every 24 hours until satiety, usually 10 to 15 minutes on each breast.[87,88] In the early weeks after birth, nondemanding babies should be aroused to feed if 4 hours have elapsed since the last nursing.[89,90] Appropriate initiation of breastfeeding is facilitated by continuous rooming-in.[91] Formal evaluation of breastfeeding performance should be undertaken by trained observers and fully documented in the record during the first 24 to 48 hours after delivery and again at the early follow-up visit, which should occur 48 to 72 hours after discharge. Maternal recording of the time of each breastfeeding and its duration, as well as voidings and stoolings during the early days of breastfeeding in the hospital and at home, greatly facilitates the evaluation process. 4. No supplements (water, glucose water, formula, and so forth) should be given to breastfeeding newborns unless a medical indication exists.[92-95] With sound breastfeeding knowledge and practices, supplements rarely are needed. Supplements and pacifiers should be avoided whenever possible and, if used at all, only after breastfeeding is well established.[93-98]

Most debates about lawyers professional responsibilities ultimately devolve into discussions of the “role” of lawyers-including the functions lawyers serve in the legal system, how lawyers should achieve those functions, and how lawyers should reconcile conflicts between competing interests that are potentially affected by their conduct. The notion of a lawyers role can be helpful for identifying principles of behavior. Thus, for example, whether attorney-client confidentiality is appropriate and whether exceptions should be acknowledged depends on the conception of lawyers from which one starts. If lawyers are to be clients sole and fully-allied champions, then strict confidentiality rules make sense. If lawyers are to be objective intermediaries between clients and society and have independent obligations to the law, exceptions seem more acceptable. The task of code drafters is facilitated when they can proceed from the assumption that a single paradigm governs the lawyers role-either a paradigm encapsulating a theory of behavior (how lawyers ordinarily act) or a paradigm encapsulating a normative ideal (how lawyers should act). When code drafters rely upon multiple paradigms (for example, lawyers should act as clients alter egos and as officers of the court), inconsistent regulatory mandates can result, and lawyers may become confused about their functions. The ABAs 1908 Canons of Ethics, which prevailed in most American jurisdictions throughout the early twentieth century, suffered from an emphasis on inconsistent ideals.1 The bar was in a state of disagreement about lawyers roles and lawyers had no point of reference for determining how to act.2 Therefore, when the ABA adopted the first concrete professional code in 1969,3 it became important for the drafters to identify an over-arching theory. By emphasizing the lawyers place in the adversarial system, the 1969 code served a forceful unifying and educational function.4 Nevertheless, an ethics codes overemphasis on a single paradigm can lead to undesirable results. It tends to produce idealized rules that either rest on counter-factual assumptions or impose obligations on lawyers that are so inconsistent with their personal incentives that violations become routine. In modern times, an exclusive focus on the adversarial ethic seems less important than in 1969 because lawyers have developed a shared sense of core obligations to clients and of the part lawyers play in the legal system. The recent history of lawyer regulation reflects a desire on the part of the bar to fine-tune the basic message of the 1969 code-to recognize more nuances than, perhaps, a single image of lawyers can accommodate.5 The task of drafting professional rules is complicated by the fact that the public perception of the bar is not unitary. Society views lawyers in many different, and sometimes inconsistent, ways. These multiple conceptions influence code drafters. In practice, the modern legal ethics codes incorporate a number of “images” of lawyers as bases, or catalysts, for disciplinary rules. Relying on these images has consequences for how the codes treat lawyers and, as a theoretical matter, for how the rules should be drafted. Part I of this Article identifies some of the different images of lawyers that exist6 and the ramifications of emphasizing each image in the professional codes. Initially at least, this Article takes no position on the accuracy of the images in describing lawyer behavior-for example, whether lawyers act in a selfinterested manner or whether lawyers feel bound by unenforceable professional norms. As a practical matter, each of the images has solid footing in some respects. That is to say, each is based on a valid characterization of how lawyers act in some situations. Part I will suggest, however, that relying upon multiple paradigms inevitably fosters schizophrenia on the part of the code drafters.

Laser hair removal, once limited to patients with dark hair, can now be effectively performed on blonde facial hair with the assistance of a topical melanin solution. According to Elizabeth Tanzi, MD, of Washington, DC, this enhanced clinical method offers promise to patients with fair hair who hope to take advantage of the long-lasting benefits of laser hair removal. Unfortunately last weekends weather forecast gave a bad report for boat fishing and consequently a lot of charter skippers cancelled the parties they had booked. Once again the weather forecasters got it wrong so charter skippers, tackle shop owners and anglers lost money and sport. There were one or two private boats out ( one was the Blue Bell II out of Amble, with Tony Urwin and his wife. Fishing just outside the harbour they had over 30 cod between 3-5lb. Private boats out of Seaham also managed to get out and, again fishing just off the harbour, they took cod to 6lb. On the subject of boats, two more charter vessels for you.

The new Coast Guard Five-Star Safety Program helps small charter boat owners prepare vessels so passengers have an improved chance of survival in case of accident. Charter boats carrying six passengers or less is big business in Alaska. This year, the Coast Guard reports more than 2,000 vessels registered as “Six-Pack” boats in the state. Combined, these small, uninspected passenger boats will carry nearly one million passengers on fishing trips, whale watches, sightseeing tours or water taxis. Many will be tourists visiting Alaska on vacation. In March, the 17th Coast Guard District Office in Juneau announced a pioneer program for small charter boats known as the Five-Star Safety Rating Program. This voluntary, Alaska-only program is designed to make sailing on this smallest-size class of passenger vessel safer. Vessel owners that implement all of the suggested safety upgrades can earn a Five-Star Safety Rating for their crafts. According to Coast Guard Capt. Ed Page, chief of marine safety in Alaska, “This program outlines a suite of recommended safety measures, and is designed to provide safety-conscious operators recognition for their efforts to provide a safer vessel for their passengers.” Lieutenant Chris Woodle, supervisor of Coast Guard Marine Safety Detachment Kenai, was in Homer May 10 when the first Five-Star sticker was earned by Robert Ward, owner/operator of the charter fishing vessel No Problem. “Theres been some concern amongst the charter fishing fleet that the smaller vessels dont have to meet the strict Coast Guard guidelines the larger charter boats are required to meet,” he said. “This non-regulatory program will help bridge the gap between the larger inspected charter boats and the uninspected six-pack charter boats, and could save lives.”
